This form is to be used to request a business continuity rehearsal. It is designed to allow you to give the minimum information required to ensure that the correct resources are made available by Wavenet for the rehearsal. For more information on how to complete the form, call or email below.

Please indicate your Network and Telecommunications requirements. Please ensure that you match with your contract schedules and indicate any changes that may be required to previously stored.
Basic Option – DDI (direct dial) facilities
Advanced Option – Do you need telecoms configuration. i.e. Hunt Groups, ACD, Call recording.
Daisy Network Services – Do you need contracted network equipment i.e. switches routers, firewalls
Please indicate whether any of your staff that may visit our premises as part of this rehearsal have any special requirements for which we need to be aware. For example: impaired hearing or vision, wheelchair access, a parking place as close as possible to the building. Daisy can then consider any specific health and safety arrangements that might be appropriate
